Kaiser is a Health Maintenance Organization (HMO) that provides patient services, hospitalization, supplies and prescription drugs through its own network of doctors, hospitals and other Kaiser-affiliated health care facilities. Kaiser covers your medical expenses only if you go to a Kaiser provider or facility. You are also covered if you have a life-threatening emergency when you are outside of a Kaiser service area. Once enrolled, you are required to select a Primary Care Physician (PCP) to manage your care within Kaiser's network and to refer you to specialists. The Kaiser plans are available to employees located in the state of California. Ambarella offers two plans through Kaiser:
 

Kaiser HMO Plan (CA only)

  • The Kaiser HMO plan offers an all-in-one approach to healthcare, with predictable copays with no deductibles that make managing your life, your health and your budget simpler. When you enroll, you choose a primary care physician who can provide routine care as well as referrals to a broad network of specialists and facilities. There’s no deductible requirement, and for most services you pay a flat “copay” amount, rather than a percentage of the cost. Keep in mind that an HMO typically covers only in-network care except in life-threatening emergencies. HMO plans offer a comprehensive, convenient approach, but generally require that you seek care within the network.
     

Kaiser HMO High-Deductible Health HSA Plan (CA only)

  • These leading-edge programs give you more control over how, when and where you get medical care – and how much you pay for it. The HSA plan combines traditional PPO coverage with a higher deductible and a tax advantaged Health Savings Account (HSA). Here’s how it works:

    • First, you will open a Health Savings Account that you can fund with pre-tax dollars, is completely portable, and can even offer investment options

    • Ambarella then makes employer contributions into the health savings account of $750 per year for employee-only coverage, $1,500 for families

    • You can use the money at any time to help pay for eligible medical expenses, including your deductible

    • Like any of our plans, most preventive care is covered for free – with no deductible requirements and no provider restrictions
