As an eligible employee, you are provided a benefit of 1x your salary, up to a maximum of $400,000. This benefit will be rounded up to the next $1,000 level. For example, if you make $105,500, your Basic Life benefit will be $106,000. 



If death is the result of an accident, the Beneficiary will receive an additional amount equal to your Life Insurance in force. If you are dismembered (such as loss of sight in an eye, loss of a hand, foot, limb, hearing, speech, etc.), you will receive a benefit of 1x your salary, up to a maximum of $400,000. Just like the Basic Life benefit, this amount will be rounded up to the next $1,000 level. For example, if you make $105,500, your AD&D benefit will be $106,000.
