You can enroll for Pet Care through Eusoh at any time. You pay Eusoh directly; you are not able to pay for Pet Care via paycheck deduction. Eusoh only covers dogs and cats.

There are two types of costs that go into your monthly coverage: your monthly subscription cost and your share of your member services. Your monthly subscription fee is $15. Your monthly shared costs vary because of the amount of expenses submitted by our community. Every member commits to a maximum of $63/month. Whatever isn't used gets credited to your next month's payment. On average our members pay $40-$45 per month.

Once you register with Eusoh you’ll pay an initial $48 deposit so that funds are available for sharing. When expenses are submitted to the site funds are drawn from your deposit. The maximum that can be withdrawn each month is $48.
After an initial 30- day waiting period, you can start submitting expenses. There is an annual out-of-pocket deductible of $250. Basically, after joining Eusoh, you'll cover $250 before the community shares with you. Once you hit that out-of-pocket deductible, your share for every expense is 20% with the Eusoh community covering the other 80%.
When you visit the vet, you'll pay for costs up front and then submit the receipts for reimbursement online. You can receive a maximum reimbursement of $8,500 every year.
Yes, you can visit any licensed veterinarian anywhere in the United States.
You can register with Eusoh without joining a group or putting in your financials. This will give you access to their Procedure Guide which you can use to search to see what procedures are covered and eligible to be reimbursed.

• Benefit enrollment for new employees. If you are a new employee who just joined Natus, you need to enroll for your Natus benefit coverage within 31 days of your date of hire..
• Make changes to your benefits. You can make changes to your benefits only if you are an existing employee who had a qualifying life event. Please note that you otherwise cannot make changes to your benefits and will only be able to do it during Open Enrollment.
• Update your information. If you need to update your credentials, for example, you have moved and would like to update your home address or phone number, you can do it via PlanSource.

For example, if you live in the Kaiser California service area, you will be able to choose one of the following three plan options for 2020: Kaiser California, Aetna PPO, and Aetna PPO with HSA.

HSA is always paired with a qualified high-deductible health plan such as the Aetna PPO with HSA plan offered by Natus. HSA provides a triple-tax advantage:
1. You can deposit money tax-free
2. The account will grow tax-free until you use it
3. Your withdrawals are tax-free when used on qualified expenses.

A formulary is a list of preferred generic and brand-name drugs used to identify medications, which have been approved based on their safety, clinical effectiveness, and cost. All medical carriers use such a list for Natus’ medical plans.
For 2020, Aetna’s formulary list will continue to be under Aetna Standard Plan, applicable to Aetna PPO, Aetna PPO with HSA, and Aetna EPO plans. The formulary list will change slightly for 2020. Please check the following Aetna site to learn how your medication will be covered.

Spring Health (https://www.springhealth.com/)is the administrator of Natus' mental health program. Natus pays 100% for this plan. Employees, spouses/domestic partners and dependent children ages 13 – 26 are eligible to use the new mental health services regardless of the medical plan elected.
You can continue to use Spring Health services through the end of 2021. The mental health services are available to support you and your family for all ranges of mental health challenges, from work-related stress to depression or anxiety. Below is a summary of the program overview:
• You can access Spring Health through Spring Health App via your smartphone or by logging onto Spring Health website via your laptop
• Screening: You will be asked to answer questions on the Spring Health site, so that a Spring Health Care Navigator can assess symptoms and identify underlying mental health issues
• Personalized treatment: Your Care Navigator will work with you to create a personalized treatment plan specific to your unique situation
• Care navigation: You can connect with your Spring Health Care Navigator via phone, email, or text on an unlimited basis for support. In addition, the Care Navigator will help you track your progress based on the treatment plan.
• Access to additional care: You will also have access to Spring Health’s providers, including 6 virtual or in-person visits per year at no cost to you.
Spring Health care navigators are licensed therapists, family therapists or licensed clinical social workers.

Mental health services are available to support you and your family for all ranges of mental health challenges, from work-related stress to depression or anxiety. Below is a summary of the program overview:
• You can access Spring Health through Spring Health App via your smartphone or by logging onto Spring Health website via your laptop
• Screening: You will be asked to answer questions on the Spring Health site, so that a Spring Health Care Navigator can assess symptoms and identify underlying mental health issues
• Personalized treatment: Your Care Navigator will work with you to create a personalized treatment plan specific to your unique situation
• Care navigation: You can connect with your Spring Health Care Navigator via phone, email, or text on an unlimited basis for support. In addition, the Care Navigator will help you track your progress based on the treatment plan.
• Access to additional care: You will also have access to Spring Health’s providers, including 6 virtual or in-person visits per year at no cost to you.

After you exhaust the 6 virtual or in-person visits with Spring Health providers for the year, you can:
• Continue to obtain support from the Spring Health Care Navigator via phone, text or e-mail
• Contact your medical plan and ask for mental health assistance. Your Spring Health Care Navigator can also assist you in this process.
• Utilize Employee Assistance Program
• Continue to see the Spring Health provider by paying the service fees to the provider on your own.
